Two annual plans. Standard is $350/year for the first member ($300/year each additional member); the Perio plan is $450/year ($400/year each additional). No insurance required, no waiting periods, no deductibles. Full benefits the same day you sign up.

Each additional member is $50 off the first-member price on either plan. Spouses, children under age 21, and any additional persons living at the same address.

This plan cannot be combined with any insurance policies or discount plans.
Implants and cosmetic treatment are eligible for the membership discount.
We partner with Care Credit and Cherry for monthly payment plans, including promotional no-interest options when you pay the balance in full within six months to one year, depending on the financing terms you qualify for.
Optional auto-renewal locks in your enrollment rate. If your plan lapses, you re-enroll at the then-current pricing.
Enrollment fees are non-refundable. Additional charges for services beyond the included benefits are due on the day of service.

Use it day one

There's no waiting period and no deductible. Full benefits are available the same day you sign up. If you're coming in for a first visit, we can enroll you and apply the discount to that day's treatment.

Common questions

Who can I add to my plan?: Spouses, children under age 21, and any additional persons living at the same address.
Can I use this plan with my insurance?: No. This plan cannot be combined with any insurance policies or discount plans.
Are implants covered? What about cosmetic options?: Yes and yes. Implants, veneers, Invisalign and clear aligner treatment, periodontal therapy, oral surgery, sedation, and other cosmetic care all qualify for your plan discount on non-included services.
Can I use Care Credit or other financing options with this plan?: Yes. Through Care Credit and Cherry, we offer monthly payment plans, including promotional no-interest plans when you pay the balance in full within six months to one year, depending on the terms you qualify for.
Are there any waiting periods or deductibles?: No. There are no deductibles or waiting periods. Full benefits are available the same day you sign up.
Can I use my HSA/FSA for this plan?: Each HSA/FSA is subject to individual guidelines. We can provide documentation for your costs and enrollment fees to submit to them.
Does the plan auto-renew?: You can opt in to auto-renewal to lock in your enrollment rate. If a plan expires without renewal, you will need to re-enroll at the then-current pricing. You can opt out of auto-renew before your annual date.
How do I choose between the Standard and Perio plans?: Choose the Standard Plan if your gums are healthy and you need routine preventive cleanings. Choose the Perio Plan if you have gum disease: periodontal maintenance cleanings are provided as diagnosed by your provider, and you receive a higher discount on other procedures. We are happy to help you decide at your visit.
